With all of the attention on
avoidable readmissions by the government, payers and the press,
someone forgot to tell stakeholders about the challenging nature of
dealing with a complex system of transitioning through different
care settings. While many promising practices are available, how do
you tie them all together to meaningfully reduce avoidable
readmissions? And in a time with diminishing reimbursements, how do
you do it cost-effectively?
We explore the lessons from the
early adopters and how to adapt them to different organizational
settings. Connecting the dots with different caregivers and
empowering patients and families as active participants are the
goals of this session. It is all about immediate action with
sustainable results – and the winners are the patients and their
families. Patients and their families are an underdeveloped
resource, mainly because their needs and engagement have
historically been addressed in a standardized, one-size-fits-all
manner. Customized activation of "home caregivers" will be explored
based on patient's individual ideas of independence, self-mastery
and degree of support at home.
Major reductions in
readmission rates require new relationships and strategies to
interact with new partners. Hospitals will need to select key
partners to address avoidable readmissions in a more comprehensive
manner. Selection of these partners will need to be based on local
needs, readmission patterns, experience and interest.

Bruce Spurlock, M.D. is President & CEO of Convergence
Health Consulting, Inc., a boutique management consulting
firm committed to leading improvement in health care. Dr.
Spurlock works with physicians and health care executives to
create state-of-the-art clinical and operational management
systems. Dr. Spurlock is Chair of the CHART Board, a
groundbreaking initiative of hospitals, purchasers, health
plans and consumer groups to produce a voluntary hospital
performance report in California. He also leads large
quality collaboratives in California, Washington and Florida
designed to accelerate the implementation of evidence-based
practices. Currently, he is an Adjunct Associate Professor
with Stanford University. He is a nationally known speaker
on health care topics ranging from health care trends to
redesigning specific operational units.
Prior to
developing his national consulting practice, Dr. Spurlock
was the executive vice president for the California
Healthcare Association (CHA) where he served as CHA’s expert
in physician group relations, health policy, and quality and
performance measurement. He also practiced internal medicine
as a senior physician with The Permanente Medical Group,
Inc., for 10 years, where he led patient education services
and developed shared decision-making tools, clinical
pathways and guidelines, RN advice center protocols and easy
access ambulatory services. During his clinical career Dr.
Spurlock was an Assistant Clinical Professor for the
University of California, Davis and participated on the
American Hospital Association’s Quality Leadership Team. In
1997 he was appointed by California Governor Wilson to the
Managed Health Care Improvement Task Force, is published in
local, state and national journals and has served as a
trustee on the California Medical Association’s Board of
Trustees.
