Health Plan Readmissions Strategies: Contracting and Care Management Approaches 
 
Overview
Hospitals and applicable physicians and other providers have been focusing on reducing preventable readmissions, significantly due to Medicare policy and payment changes in this regard. Health plans have always had a vested interest in managing readmissions, and with the advancement of available tools, health care reform, and other environmental changes, plans are becoming increasingly proactive in launching new readmissions management initiatives with their partnering provider organizations.

Health plan readmissions management encompasses a variety of approaches, that often incorporate new provider payment structures, data tracking, education and engagement. Some initiatives are designed for specific care delivery settings, while others are system-wide programs.

  4. Ascertain a medical home approach that creates a segmentation methodology facilitating population health analyses, building a primary care physician attribution model, and executes a contracting strategy leveraging these elements within the Patient Centered Medical Home.
  5. Engage in interactive learning through online question submission, attendee feedback and opportunity for follow up questions, and networking with attendees, faculty and other professionals through a dedicated LinkedIn group

  Doctor Gifford is a frequent speaker on innovation in health care at national conferences and leadership events. He has executive experience in the information technology sector as well as on both the provider and payer side of health care services.

Prior to joining Regence, Dr. Gifford co-founded QuickCompliance, a health care Internet media company now part of the Discovery Channel, and served as director of product management at ChannelPoint, a software company founded by engineers from Sun Microsystems now controlled by Trizetto.

Dr. Gifford has authored a variety of publications in clinical research and health care services, and has created online training programs for many large organizations, including CMS and the Department of Defense. He received his clinical training at the University of Washington Hospitals after receiving his M.D. degree from the University of California at San Diego.

Doctor Robert (Bob) Herr has twelve years leadership experience in medical healthcare cost containment. Doctor Herr has served as a Medical Director with Regence since July 2007.

He is board certified in internal medicine and emergency medicine, and is a Certified Physician Executive. Doctor Herr received his Medical Degree from Yale University, and his MBA from the University of Utah - David Eccles School of Business.

Dr. Brian Wolf is the Senior Medical Director for Health Analytics and Utilization Management, with additional oversight of Physician Profiling and Audit and Recovery, at Blue Cross Blue Shield of Rhode Island. He also serves as Physician Advisor for Wellpoint with responsibility for Utilization Management of the Unicare product.

Previously he was site Chief of Surgery and Surgical Oncologist, as well as Division Chief of General Surgery for the Cambridge Health Alliance from 2002-2007. From 2000-2002 he served as Surgical Oncologist and Director of Surgical Services for Tri-County Medical Associates From 1989 to 1999 he was Surgical Oncologist and Chairman of Physician Executive Council for Harvard Pilgrim Health Care of New England.

Dr. Wolf received his Doctor of Medicine from Boston University School of Medicine, Boston, Massachusetts and is Board Certified in General Surgery..
