Coordinated Care: Everything You Wanted To Know 
   
Overview
Coordinated care may well prove the linchpin of ACOs and other cost-control and quality-enhancing programs that are the products of the Affordable Care Act. The increasing number of Americans with chronic conditions such as diabetes, asthma and congestive heart failure is contributing to a relentless increase in the cost of delivering healthcare. Moreover, many providers face financial penalties if they do not reduce patient readmissions and increase patient satisfaction. A more closely managed, patient-centric care model may be the only way to bring such cost trends to heel.

However, what does that mean for California? What do these programs consist of, how are they deployed, and what are their results? Are medical homes the only way to deploy a coordinated care program, or are there other solutions?

Wells Shoemaker, M.D.
Wells Shoemaker, M.D.,
Chief Medical Officer, California Association of Physician Groups
  Dr. Shoemaker is the medical director of the California Association of Physician Groups (CAPG), which includes more than 150 integrated medical groups and IPAs up and down the state, caring for approximately 18 million Californians. He also co-chairs the California Quality Collaborative (www.calquality.org).

Dr. Shoemaker completed his pediatrics training at Stanford University in 1975 and practiced in the California coastal community of Watsonville for 17 years, during which time he opened a Level II nursery, a lactation program, and other community pediatric programs. He subsequently taught in the family practice residency program at Natividad Hospital in Salinas before accepting the position of initial medical director of the Medi-Cal Managed Care program for Santa Cruz and Monterey Counties in 1995. He moved to the private sector in 2000 as medical director of Physicians Medical Group of Santa Cruz County and subsequently to his statewide role with CAPG in 2006.
 


Eve Gelb
Eve Gelb,
Senior Vice President of Healthcare Services,
SCAN Health Plan
  Eve Gelb has over two decades of experience in healthcare. In her role as senior vice president – healthcare services for SCAN, Eve is responsible for all healthcare services activities of the nation’s third-largest not-for-profit Medicare Advantage Plan, including care management, care coordination, quality management, and utilization management.

A member of the SCAN team since 1997, Eve has served SCAN as manager of business development, director of the geriatric practice innovation (GPI) unit (where she helped build SCAN’s care management and disease management programs), director of the project management office for SCAN and vice president of healthcare services administration, where she was responsible for quality management, disease management and clinical software.

Prior to joining SCAN, Eve worked as a health educator at Texas Children’s Hospital, setting up asthma education programs and primary care networks for low-income children.

She earned a master of public health degree at Tulane University and bachelor of arts degree in anthropology at the University of California, Los Angeles.
