One thought leader in Population Health News recently defined population health management as a program is "strives to address health needs at all points along the continuum of health and well-being through the participation of, engagement with and targeted interventions for the population. The goal of a population health management program is to maintain or improve the physical and psychosocial well-being of individuals through cost-effective and tailored health solutions." Another defined it as "the ability to define and understand the health status of every individual patient and proactively deploy medical resources to support those patients, whether it is to push resource to them where they are, or effectively connect them to the optimal resource in a patient specific manner, accelerated by technology.”

Population Health has gravitated to become a central component of the delivery of healthcare in the 21st century. It is critical for leaders, clinicians and staff of healthcare organizations to have a vision going forward on how to best incorporate population health into their approach - sharing from insights, innovations, best practices, strategies and experiences from national leaders involved with population health.

Susan Pantely

Susan E. Pantely, FSA, MAAA Principal, Consulting Actuary
Milliman, Inc.

  Susan is a principal and consulting actuary with the San Francisco office of Milliman. She rejoined the firm in 2002. She previously worked for the New York office of Milliman from 1988 through 1999.

Susan provides actuarial and consulting services to a broad range of clients, including Blue Cross/Blue Shield plans, HMOs, commercial insurers, government agencies, and healthcare providers. Her work includes rate development, provider contract review, reserve certification, capitation development, Medicare risk feasibility studies, HMO start-ups, HMO due diligence, and development of risk-sharing and reimbursement arrangements for physician groups, PHOs, and other integrated delivery systems.

In addition, Susan has extensive experience with the valuation, financial analysis, and projection of healthcare services for several state public health insurance (Medicaid) programs. Prior to rejoining Milliman, Susan worked at Andersen LLP and Ernst & Young, LLP.

Susan serves on the Society of Actuaries' Education and Examination Committee, and as chair of the SOA's Health Section Council. Susan is also a member of the American Academy of Actuaries' Health Care Quality Workgroup. 

Her presentations and publications include: "An Electronic Prescription for Health Care Efficiency" (Society of Actuaries HealthWatch, January 2009) "Employer-Paid Nonmedical Costs for Patients With Diabetes and End-Stage Renal Disease" (Preventing Chronic Disease, July 2006); "How Savvy are Employees (and Employers) about Their Health Benefits?" (Milliman Benefits Perspectives, Winter 2008); "Medicaid Program Redesign: The Long-Term Care and Developmentally Disabled Programs" (report for Wyoming Health Care Commission, September 2006, co-author). Susan received a BS in Mathematics from the University of Pittsburgh.

Vicky Parikh is an experienced Public Health Administration physician with experience in managed care, grant administration, physician relations, and network development utilizing strong IT understanding and application of predictive analytics for the advancement of innovative access for patients of diverse populations. He led Successful formation of 5 Centers for Medicare and Medicaid Services (CMS) approved Accountable Care Organizations in Maryland, Virginia and the DC region.

As Executive Director of Reliance Health, LLC since 2012, Dr. Parikh has Supported Reliance start-up business plan development and execution of growth initiatives. He has lead teams and resources for care management planning and implementation and transition of care and utilization management, and managed a multi-million dollar budget. During his tenure to date he has led application, approval, formation, and implementation of 5 Accountable Care Organizations (ACO) in 12 counties in Maryland, 4 counties in Virginia and District of Columbia, improving patient experience of care, health of populations, and reducing per capita cost of healthcare. He formed a strategic alliance with a national company, coordinating care for over 60k Medicare beneficiaries.

As Executive Director for Mid-Atlantic Medical Research Centers (MAMRC) since 2009, Dr. Parikh has led and directed multi-therapeutic area clinical research operations including fiscal management, study and site operations, Program Development, Regulatory, and Patient Recruitment. He has Developed strategies, plans and managed clinical studies; managed daily and proposed activities of the Research Department; and ensured compliance with local and federal regulations.

Dr. Parikh is also an Associate Director with the Shah Associates Medical Group since 2010, where he has designed programs for education and engagement of physicians in-group regarding Patient Centered Medical Home (PCMH) and Maryland Multi Payer Program (MMPP), repositioning practice as leading manager in population health, improving clinical outcomes. Created awareness and set up 21-member Community Relations council, identifying gaps in care, care issues, and reviewed performance indicators, clinical pathways and evidence-based medicine protocols. He has Worked with physicians regarding EHR meaningful use stage 1 and 2, and NCQA level 2 and 3 certification.

Doug Tynan has advocated for and practiced in integrated health care settings for over 25 years in both primary care and specialty care pediatric settings. He developed one of the first psychosocial programs for children with diabetes and a primary care practice model. He previously worked for 13 years at the Nemours/Alfred I. DuPont Hospital for Children in Delaware. He served as the training director of a doctoral internship program and as the principal investigator on one of the first Graduate Psychology Education program grants.

Before joining APA, Tynan spent 13 years with the Nemours/Alfred I. duPont Hospital for Children in Delaware, most recently as chief psychologist with Nemours Health and Prevention Services. In that post, he directed implementation of statewide initiatives to promote parenting skills, and social, emotional and physical health for children in family, primary care, child care, school and after-school settings. He developed a federally funded program that trains both pediatric and psychology residents in the assessment and treatment of child behavior problems in primary care settings, and coordinated a clinical intervention program for children with disruptive behavior and their parents.

Earlier in his career, he spent two years as director of the ADHD (attention deficit/hyperactivity disorder) Evaluation & Treatment Program with Geisinger Medical Center in Danville, Pennsylvania. From 1987-1998, he held a variety of positions with the Children’s National Medical Center in Washington, D.C.

A graduate of Boston University, Tynan earned a master’s degree in psychology at the University of Connecticut and a PhD in clinical psychology at Binghamton University. He is a licensed psychologist in Delaware and Pennsylvania, an APA fellow and a member of the Delaware Psychological Association. Tynan is also a professor of pediatrics at Jefferson Medical College and an associate professor of health sciences at the University of Delaware. He currently serves on Delaware’s State Health Care Innovation Plan workforce and financing work groups. He previously served on Delaware’s Early Childhood Council, the Head Start Partners in Excellence Program, the Coordinating Council of the Delaware Health Science Alliance and the Professional Advisory Board of the Delaware Epilepsy Foundation. He is co-editor of the APA journal Clinical Practice in Pediatric Psychology.

Bob Gladden is Vice President for the Center for Analytics at CareSource, a nonprofit Medicaid organization with over 1 million members. He leads analytics for the organization that guide its mission-driven focus.

Bob has over 30 years of experience in the fields of health care analytics, health care administration, actuarial science, underwriting and finance with major consulting organizations, hospital systems, management companies and payers. Bob holds an MA degree from Bowling Green State University.

Cathy Meade is Director for Healthcare Analytics at CareSource, a nonprofit Medicaid organization with almost 1 million members. She currently leads the health care analytic efforts for the organization, with a focus on diagnostic and predictive analytics that maintains a strong practical sense of helping the organization achieve its mission-driven focus.

Cathy has over 20 years of experience in the fields of health care analytics, business process improvement, finance and IT within the health care, technology and banking industries. Cathy holds a BA degree in business from Capital University.

Randy Krakauer, MD

Randall Krakauer, M.D., is Aetna's National Medical Director for Medical Strategy . Dr. Krakauer graduated from Albany Medical College in 1972 and is Board Certified in Internal Medicine and Rheumatology. He received training in Internal Medicine at the University of Minnesota Hospitals and in Rheumatology at the National Institutes of Health and Massachusetts General Hospital/Harvard Medical School, and received an MBA from Rutgers.

Dr. Krakauer is a fellow of the American College of Physicians and the American College of Rheumatology and Professor of Medicine at Seton Hall University Graduate School of Medicine. He is past chairman of the American College of Managed Care Medicine.

Dr. Krakauer is a recognized leader in geriatric care and has more than 30 years of experience in medicine and medical management, has held senior medical management positions in several major organizations.


Benjamin Littenberg, MD

  Dr. Littenberg is the Henry and Carleen Tufo Professor of Medicine, Professor of Nursing, and Director of General Internal Medicine at the University of Vermont. He received his medical degree from Case Western Reserve University and trained in internal medicine at Hartford Hospital in Connecticut. He was a Robert Wood Johnson Clinical Scholar at Stanford University before becoming Assistant Professor of Medicine at Dartmouth. Following an appointment as Associate Professor of Medicine at Washington University in St. Louis, he assumed his present position in 1999.

Dr. Littenberg practices and teaches General Internal Medicine in Burlington, Vermont. His research interests center on technology assessment and quality improvement. Recent projects include new ways to measure quality of care in cancer, novel strategies for reporting test results to patients, better approaches to antibiotics in sinusitis, safety improvements in outpatient prescriptions, and strategies to address health literacy.

In Vermont, Dr. Littenberg has been active on the Board of the Vermont Program for Quality in Health Care. He has developed statewide registry-based approaches to quality and safety improvement with the Vermont Breast Cancer Surveillance System and as Principal Investigator of the NIH-funded research grant for the Vermont Diabetes Patient Engagement System from which Patient Engagement Systems® evolved.


Joseph Vassalotti, MD

  Dr. Vassalotti is the Chief Medical Officer of the National Kidney Foundation (NKF) and Associate Clinical Professor of Medicine, Division of Nephrology, at Icahn School of Medicine at Mount Sinai. He received his medical degree with Distinction in Research from the SUNY Stony Brook School of Medicine and completed a Fellowship in Clinical Nephrology at Johns Hopkins Hospital. At NKF, the major focus is implementation of the U.S. KDOQI Clinical Practice Guidelines in primary care. He is an investigator for the NIH-sponsored clustered practice randomized trial entitled, evidenced-based primary care for CKD. He also serves as the Lead Clinician Consultant for the Fistula First/Catheter Last Quality Improvement Project of the End Stage Renal Disease Network Coordinating Center funded by the Centers for Medicare and Medicaid Services. Dr. Vassalotti is published in numerous peer-reviewed journals. Dr. Vassalotti has been featured in Best Doctors in America.
