Overview
Population Health Management holds significant potential to help redefine care management and address the increasing fragmentation in health care. As the field of population health grows, physicians continue to question how to use this approach to improve the care of the individuals in their practice. It is crucial at this juncture that stakeholders in population health demonstrate the value of this approach to physicians on the front lines of medicine. Many practicing physicians have questions or confusion regarding how to apply population health data and interventions.

Ascension Health's SmartHealth is the ERISA based health insurance provider for 95,000 Ascension employees. SmartHealth covers around 210,000 employees in 70 different hospitals owned by Ascension across the United States. The principles of population health have been applied to Ascension Health's employee population, generating a series of integrated wellness, rising risk, and complex case management interventions. In addition, to improving the health of the employee population, this intervention strategy of initiatives have been used to educate physicians illustrating the value and importance of population health based approach to improving the care of their patients and improving practice revenue.

Ascension Health's SmartHealth case study regarding the health status and outcomes of the employee population and associated interventions provide an illustration of the population health process and how it applies to physician practices. The care study reviews an analysis of a population's health, the resulting interventions, measurements tracked, variety of contract models that might be associated with a practice setting, and the applicability of these interventions to enhancing the success of the physician achieving the contractual goals.

  Michael Kobernick, MD, MS, FAAEM, FAAFP is Chief Medical Officer of Ascension Health's SmartHealth. SmartHealth is the ERISA based health insurance provider for 95,000 Ascension employees. SmartHealth covers around 210,000 employees in 70 different hospitals owned by Ascension across the United States. Dr. Kobernick has overall responsibility for all care performance programs including the departments of population health, utilization management, quality, and pharmacy.

Dr. Kobernick completed his undergraduate and medical education at Michigan State University. Prior to joining SmartHealth, Dr. Kobernick was Medical Director of the Macomb ED. In 2007, the University of Detroit-Mercy recognized him as Alumni of the Year and in 2012, he was recognized as a “Top Doc” in the Detroit area. He recently enrolled in the Master’s program in Health Policy at Jefferson Medical College.
