As patient centered medical homes emerge and evolve in an environment of health reform, careful consideration and evaluation must be given to systems and health plans that have developed a successful track record in this arena. The Geisinger Health System and Blue Cross Blue Shield of Rhode Island have received national recognition for their patient centered medical home initiatives.

  Diane Littlewood RN, BSN, CDE, is a Regional manager for nurse case managers with the ProvenHealth NavigatorSM program with Geisinger Health Plan. In her role, she is responsible for the start up and maintenance of the medical home program for those sites enrolled in the program in the north central region. She also serves as a liaison between the Geisinger Health Plan and Geisinger Community Practice.

Ms. Littlewood received her Bachelors of Science in Nursing from Misericordia University and is a Certified Diabetes Educator and Certified Tobacco Cessation Counselor. Her experience includes medical surgical nursing as well as oncology, cardiology, gastroenterology, and ob/gyn. More recently, she has been involved in case and care management as a case manager, preceptor, and team leader in care coordination.

Ms. Littlewood is president of the Danville chapter of CMSA and is involved in community education programs as well as educational programs for case management staff.

  



Joann Sciandra

  Joann Sciandra RN, BSN, CCM, is a Regional manager for nurse case managers with the ProvenHealth Navigator SM program with Geisinger Health Plan. In her role, she is responsible for the start up and maintenance of the medical home program for those sites enrolled in the program in the eastern region. She also serves as a liaison between the Geisinger Health Plan and Geisinger Community Practice.

Ms. Sciandra received her Bachelors of Science in Nursing from Wilkes University and is a Certified Case Manager. Her experience includes nursing practice in the intensive care unit as well as geriatric clinic coordinator. More recently, she has been involved in case and care management as a case manager, preceptor, and team leader in care coordination.

Ms. Sciandra is a member of the CMSA and involved in community education programs as well as educational programs for case management staff.

Ms. Abbott is President of MEDai, Inc., an Elsevier Clinical Decision Support company, and the industry leader in predictive analytics. As President, Ms. Abbott provides leadership to position the company at the forefront of the healthcare industry. Ms. Abbott’s oversight of strategic plans advance the company's mission and objectives, to promote revenue, profitability and growth as an organization. Prior to her role as President, Swati served as MEDai’s Senior Vice President. As Senior Vice President, she was responsible for establishing key alliances that enabled remarkable growth, enhanced capabilities and industry recognition. Through focused product development and joint marketing ventures, Ms. Abbott has ensured that MEDai solutions meet the critical needs of their clients, including providers, hospitals/IDNs, payers, and IPAs. During her tenure at MEDai she has become a regular speaker in the predictive modeling/care management circuit. Ms Abbott is also on the Editorial Board for Predictive Modeling magazine, and is a regular contributor to content.

Ms. Abbott first joined MEDai as Vice President of Business Development in 2004, bringing with her a decade of expertise in healthcare analytics software in the provider and payer market segments. As a respected industry leader, Swati has been quoted in several industry publications regarding the use and future of predictive analytics, the state of healthcare data and quality improvement initiatives. She has presented topics related to predictive modeling, medical home and healthcare analytics at many industry conferences across the US each year.

Prior to MEDai Ms. Abbott was Managing Director for the Medical Management Strategic Business Unit at ViPS, Ms. Abbott provided leadership for the development of strategic solutions for the healthcare payer clients for medical management data warehousing and HEDIS reporting. She also held the title of Director, Product Management, Services and IT of HealthObjects Corporation in Baltimore, MD. As a startup company her role at HealthObjects focused on working with beta customers, managing product releases, and identifying problems/gaps with the product and proactively resolving them. Ms Abbott was also involved in leading SoftMed Systems, Inc,, to create and provide solutions for hospitals and IDNs in the key areas of medical chart abstraction, QA, UM, Risk Management, and regulatory reporting. Ms. Abbott holds a M.S. in Computer Science and a B. S. in Physics from Delhi University.

  As Director of Practice Innovation within the Provider Relations division at Blue Cross & Blue Shield of Rhode Island,Andrea plays an integral role in formulating the plan’s Patient Centered Medical Home (PCMH) strategy, including the development of its PCMH design and roll out to primary care practices.

In addition to overseeing transformation efforts pertaining to PCMH, Andrea’s team is also involved in promoting the ‘meaningful use’ of Electronic Health Records through the development of programs which provide support, both financially and in kind.

Andrea received her Bachelor’s degree from Providence College and MBA from Bryant University. In addition to working at BCBSRI, Andrea also serves as a Special Lecturer at Providence College. Andrea is a lifelong Rhode Islander and resides there with her husband and two daughters.

  Vince Kuraitis JD, MBA is Principal and founder of Better Health Technologies, LLC. BHT consults to companies in developing strategy, partnerships and business models for chronic disease management and eHealth applications delivered in homes, workplaces, and communities. BHT’s clients — both established organizations and early-stage companies — include: Intel Digital Health Group, Philips Electronics, Amedisys, Joslin Diabetes Center, Ascension Health System, Samsung Electronics, Siemens Medical Solutions, Medtronic, Varian Medical Systems, Disease Management Association of America, and many others.

Vince brings 25 years health care experience in multiple roles: President, VP Corporate Development, VP operations, management consultant, and marketing executive. His consulting and work projects span 100+ different health care organizations, including hospitals, physician groups, medical devices, pharma, health plans, disease management, e-Health, IT, and others. Vince speaks frequently at industry conferences and corporate events. He has been the opening keynote speaker at the Healthcare Unbound conferences between 2004 and 2010 and has spoken at about 35 conferences in the past 3 years. He has experience leading strategic planning retreats for Boards and physicians.

Vince’s experience includes: Principal, Better Health Technologies; President, Health Choice (medical call center), VP Corporate Development and VP Specialty Operations, Saint Alphonsus Regional Medical Center; Regional Director of Marketing, National Medical Enterprises (hospital chain with 100 facilities); Senior Consultant, Amherst Associates, national health care management consulting company. His education includes MBA and JD degrees from UCLA, and a BS degree in business administration from USC.

Vince is on the editorial advisory boards of Disease Management & Health Outcomes Journal, Population Health Insider, Medical Home News, and the International Disease Management Alliance. He serves on the ASTM International E31 Technical Committee on Healthcare Informatics (CCR). Vince is a member of the California State Bar (inactive). He also is on advisory boards for several early stage companies.
 


 

Brian Klepper, PhD

Managing Principal of Healthcare Performance Inc., a business development practice based in Atlantic Beach, FL, Brian is a nationally prominent health care analyst, adviser, commentator and entrepreneur with broad experience and relationships.

An active author and speaker, Dr. Klepper has provided health care commentary to CBS Evening News, the Wall Street Journal, the New York Times and the Washington Post, and has published articles in Kaiser Health News, Medscape, The New England Journal of Medicine, Modern Healthcare, Business Insurance, HealthLeaders and newspapers nationally. He is a regular columnist on two of the most widely read expert health care blogs: The Health Care Blog, hosted by Matthew Holt, and Health Policy and Marketplace Review, hosted by Bob Laszewski.

He is an Aspen Institute Health Forum Fellow, serves on the Board of the Lundberg Institute, is an Advisory Board member for the Patient-Centered Primary Care Collaborative, and is one of two non-physician Board members of the Consortium for Southeast Hypertension Control (COSEHC), a physician group dedicated to translational medicine for vascular disease. He is also an advisor to the Center for Value Health Innovation.

  Kathryn Swink, is a Research Associate with the  National Association of Public Hospitals and Health Systems and the National Public Health and Hospital Institute.
Kathryn Swink joined the NAPH research team in October of 2009.

Previously, Ms. Swink worked as a maternal and child health researcher for the University of Florida’s School of Medicine and conducted pediatric HIV/AIDS research at Children’s National Medical Center in Washington, DC. She holds a B.S. degree in Chemistry and a Master of Public Health degree, with a concentration in Epidemiology, from the University of Florida.
  





Sam JW Romeo, MD, MBA

  Sam Romeo, MD. MBA is a nationally recognized health care administrator and physician with over 40 years of experience. . He received his B.S. degree from St Mary’s College, California, M.D. degree from St Louis University School of Medicine, and M.B.A. from National University. With the integrity he exudes and the warmth in his handshake, even his barber doesn’t know that he has served as the President/CEO of medical groups as large as 3,000 physicians strong, has held senior faculty and deanship positions at the USC School Of Medicine, the Medical College of Wisconsin, and St. Louis University School of Medicine, and has been the CEO and Medical Director of Health Maintenance Organizations in California, Florida, and New York. He continues to be tirelessly committed to improving health care on a national level and participates in such organizations as the Accreditation Association of Ambulatory Care (AAAHC) which insures quality standards are met by medical organizations. He has served on the AAAHC Board of Directors, AAAHC Institute for Quality Improvement (IQI) Board of Trustees, and as a methodologist in the American Medical Association (AMA) Physician Consortium for Performance Improvement which is developing evidence-based performance measurement sets for physicians to enhance the quality of patient care. From its inception, Dr Romeo has been a member of the Performance Measures Advisory Group of the CPT Editorial board creating CPT Category II Performance Measure codes. He has lectured, been published, and consulted around the country on medical ethics, occupational medicine, managed care, family practice, outcomes measurement that reflect the quality in health care, quality improvement, and the role of academic health care centers in the managed care environment. He has recently published a book “We the Patients” which is available on Amazon.com.

  Shahram Ebadollahi, Ph.D. is Manager, Healthcare Transformation Research Group; Program Manager, Healthcare Transformation; and Founding co-chair, Medical Informatics PIC at IBM T.J. Watson Research Center.  In these capacities, Dr. Ebadollahi manages a group of scientists and physicians conducting cutting edge research in the broad area of health informatics, especially applications of data mining, machine learning, advanced visualization, and simulation to healthcare applications and comparative effectiveness research.
