Social Determinants of Health: Turning Potential Into Actual Value

Did you know that social determinants of health have as big an impact on member health outcomes as health behaviors and medical care combined?

Social determinants of health (SDOH), including information on relatives and associates, assets, trends over time, neighborhood and household characteristics and more, can directly be used to improve risk stratification and care management initiatives. But first, it’s important to identify what SDOH is and how to effectively use it to get the most value for your members and organizations.

Please join us Wednesday, April 18, at 2 p.m. Eastern as we address elements of success for SDOH and leveraging SDOH to attain your quality goals while managing cost and enhancing member experience in the HealthcareWebSummit event: Social Determinants of Health: Turning Potential Into Actual Value, co-sponsored by LexisNexis Health Care with Erin Benson and Rich Morino.
