Predictive modeling techniques can be used by health plans to appropriately stratify their population and to target secondary prevention programs based upon a member's risk profile. Application of predictive modeling for proactive case management improves productivity of case reviewers and improves case open rates. Risk assessments can include evaluations against national standards of quality to provide a comprehensive analysis of clinical and financial risk for a member or group.
OVERVIEW
Chronic disease burden accounts for an estimated 75% of all healthcare costs; as much as 70% of these costs are directly influenced by individual behavior. Addressing these critical factors requires sophisticated approaches for the measurement and monitoring of population health, morbidity burden and care patterns. Successful management of chronic disease requires that effective care is encouraged and that "secondary prevention" be supported to avert or minimize the negative outcomes often associated with chronic disease.
Predictive modeling techniques can and should be used by health plans to appropriately stratify their population and to target secondary prevention programs based upon a member's risk profile. Predictive modeling can also be used to deliver targeted services to promote wellness and to help patients more effectively manage their disease. Predictive modeling can also support "gaps in care" analysis in order to identify patients that are not receiving adequate primary or secondary preventive services.
