| Health Reform is happening at the local level even more rapidly than the federal rules can be launched. Integration of various aspects of health care is occurring in this respect: with single specialty initiatives like Geisinger's program that bundles payments based upon severity, to larger efforts by Dean Health System and Baylor health system to offer a system-wide Accountable Care Organization model for their delivery system, to compete and differentiate themselves in shifting markets. At the same time health plans are experimenting with bundled payments as part of their performance contracting strategy to entice physicians and hospitals into working more closely together. The difference between the IPAs and PHOs of the past ,and the ACOs for the future, is that in addition to clinical integration of provider services, we are now seeing reimbursement integration offering an additional revenue above billed charges for those organizations that can make their care system more efficient in resource use and more effective in clinic outcomes. |
