Overview
In the tenth year of its nationally renowned program, Blue Cross Blue Shield of Michigan has designated approximately 4,690 physicians in 1,700 practices across Michigan as patient-centered medical homes. Physician practices that earn Blue Cross PCMH designation provide team-based care centered around each patient’s individual needs. The program is located in 81 of the 82 Michigan counties that have a PCP, and delivers care to over 1.35 million Michigan residents. CMS has determined that Blue Cross PCMH-designated physicians meet one component of the criteria for the Merit Based Incentive Payment System under the national Medicare Quality Payment Program.

The PCMH initiative is the largest PCMH program in the country, and is part of Value Partnerships, an array of collaborative initiatives among physicians, hospitals and Blue Cross. Blue Cross Blue Shield of Michigan provides and administers health benefits to more than 4.6 million members.

An updated, actuarial-certified analysis concludes that over nine years, the Blue Cross PCMH care model has led to $626 million in costs avoided. In comparing Blue Cross PCMH-designated practices to non-designated practices, data from 2018 show Blue Cross PCMH-designated practices had a 27-percent lower rate of adult hospital admissions for common conditions that respond to office-based care. Numerous other metrics studied indicate higher quality care, more preventive care and reduced costs for Blue Cross PCMH patients.

  Amy McKenzie, MD, MBA, is Medical Director, Value Partnerships at Blue Cross Blue Shield of Michigan. She has served with Blue Cross Blue Shield of Michigan since 2013, previously as Associate Medical Director Quality and Population Health, and as Physician Consultant, PPO and Care Management before that.

Dr. McKenzie is a Past President (May 2017 – July 2018) of the Family Medicine Foundation of Michigan. She received her Doctor of Medicine from Northeast Ohio Medical University and served her residency in the Family Medicine Residency Program at St. John Providence. Dr. McKenzie also received an MBA from the Isenberg School of Management, UMass Amherst.

Lisa Rajt* is a Health Care Manager for the Value Partnerships Program at Blue Cross Blue Shield of Michigan. She works on the award-winning Patient Centered Medical Home program, and the expansion of care management to primary care and oncology settings.

In addition, Ms. Rajt develops initiatives to improve the quality of care for women. Prior to her work in Value Partnerships, Ms. Rajt worked in the Blues' Social Mission area, where she focused on grant-making and community development.

She holds a Masters of Social Work in Community Organizing and a B.A. in Psychology and Communications, both from the University of Michigan.
