Health Reform is happening at the local level even more rapidly than the federal rules can be launched. Integration of various aspects of health care is occurring in this respect, with prominent hospital systems launching single specialty initiatives to bundle payments based upon severity as well as larger efforts to offer a system wide Accountable Care Organization model for their delivery system, to compete at differentiate themselves in shifting markets.

At the same time health plans are experimenting with bundled payments as part of their performance contracting strategy to entice physicians and hospitals into working more closely together. The difference between the IPAs and PHOs of the past ,and the ACOs for the future, is that in addition to clinical integration of provider services, we are now seeing reimbursement integration offering an additional revenue above billed charges for those organizations that can make their care system more efficient in resource use and more effective in clinic outcomes.

  William J. DeMarco MA CMC is the President & Chief Executive Officer of Pendulum Health Development Corporation, and DeMarco & Associates, Inc., a national, independent healthcare consulting firm specializing in healthcare delivery system redesign and transformation. The firm specializes in research and strategy development, organization development of successful provider owned enterprises and marketing management including product design and promotion. Mr DeMarco is recognized as a leader in the research, design and implementation of community based healthplans. Since his involvement in several startup health plans in Minnesota in the early 1970s Bill and his team of management consultants clinical specialists and reimbursement analysts have assisted employers and physicians in developing better relationships with insurers up to and including developing local solutions to deliver and finance care.
