Overview
The scale of health care transformation around the country is impacted dramatically when a national healthcare organization undergoes significant initiatives involving the delivery of integrated care through accountable care arrangements.

Tenet Healthcare, a leading national healthcare services company, has developed a range of successful collaborations with physicians and payers, designed to capitalize on the physician-led organizations that have developed robust clinical programs to improve care delivery across the service continuum. One such major initiative is a recently announced  collaboration with Blue Cross and Blue Shield of Texas to offer an innovative, statewide accountable care organization (ACO) model to deliver improved, sustainable patient care and help manage costs through Tenet's Integrated Care Networks.

Tenet's new model of healthcare delivery is designed to improve outcomes in three key categories: quality of care, patient experience and satisfaction and cost efficiency. It will reach those goals by pursuing strategies including:
Early identification of disease and illness through effective management and coordination of patient care; Use of advanced technology and support services to make more informed decisions and facilitate the patient and provider relationship and transitions in care; Implementing an alternative or non-fee-for-service payment arrangement, and
Lowering cost trends by coordination among payers and providers, without limiting medically necessary services in order to enable the delivery of more affordable healthcare products.

Conifer Health Solutions is a healthcare services company helping organizations strengthen their financial performance, transition from volume to value-based care and enhance the consumer and patient healthcare experience—with the goal of ultimately improving the quality, cost and access to healthcare. Conifer Health's Value-based Care division is supporting applicable Tenet ACO projects by identifying care improvement, efficiency and service opportunities.

With the acquisition of InforMed Health Solutions in 2012, Megan now leads a nationwide operation of value-based care solutions providing Clinical Integration, Population Health Management and Financial Risk Management for more than 300 organizations including IPAs, commercial and Medicare ACOs, clinically integrated organizations, full-risk bearing providers, hospitals, health systems, self-funded employers, Taft-Hartley plans and federal/state employer plans.

Megan served as president of Conifer Health’s Value-Based Care since its formation in 2012, and served as CEO of Cap Management Systems for more than 13 years. Megan was a driving force within Tenet leading several key initiatives tied to health care reform and supporting the formation of clinically integrated organizations throughout the country.

Megan has 20 years of risk-based hospital and physician practice operational expertise, and created a highly successful model for physician and hospital alignment in California, including services and technology infrastructure.

Megan received a Bachelor of Arts, from Boston University and a Masters of Business Administration, from Pepperdine University Graduate School of Management.

Conley Cervantes is AVP, Integrated Care Networks for Tenet Healthcare. Conley is based in Dallas and has over twenty years of experience and increasing responsibility with Tenet in managed care and related activities, including integrated care initiatives.

Conley received her undergraduate degree from the University of Nebraska-Lincoln.
